How do i add my sons father to his birth certificate?

When my son was born, his father left the hospital. He left when the nurse came in for us to sign his birth certificate and the paternity paper work. So i ended up not puting him on his birth certificate and so now its blank where his father is supposed to be. Does anyone know how i can add his father and what paperwork i need to fill out. Does it cost money also?
